Nurse Navigator Breast Health
Department: Tumor Registry
Daily Work Times: 8:00am-4:30pm
Shift: Days
Scheduled Bi-Weekly Hours: 80
Position Summary:
Responsible for the coordination of breast healthcare when a patient accesses McLaren Bay Region system. Oversees, guides, and follows up on clinical treatment plans, health promotion, and evaluations.
Essential Functions and Responsibilities:
1. Acts independently, consistently exercises discretion and judgment in performing work which is predominantly intellectual and varied in nature.
2. Collaborates with the health care team to plan and evaluate treatment options.
3. Navigates patients through the health care system including individual and family education.
4. Prepares and presents periodic outcomes reports for assigned patient population.
5. Coordinates, develops and evaluates patient and family education.
6. Develops relationships and outreach programs with oncologists, primary care physicians and surgeons.
Qualifications:
Required:
- Graduate of an accredited school of nursing and holding a current license to practice in the state of Michigan.
- BLS issued by American Heart Association or Red Cross Certification.
· Three to five years clinical experience with surgery, oncology, and health promotion
· Bachelor Degree in Nursing.
· Prior Experience as a Breast Health Navigator
· Breast Nurse Certification within twelve (12) months.
Knowledge, Skills, and Abilities:
· Demonstrates excellent customer service skills.
· Computer skills in Microsoft Word, Power Point and Excel preferred.
