Position Summary:
The Practice Coach is responsible for facilitating physician and practice processes to ensure patient data is reviewed and completed at a population level for chronic diseases and preventive services. Provides practice support for reporting quality measures across all payors. Assists the Quality Performance Specialists in providing training to practices in LEAN processes and disease registry to meet Patient Centered Medical Home (PCMH) and Neighbor (PCMHN) guidelines.
This is a hybrid position with requirements to come onsite as scheduled, Lansing, MI.

Qualifications:
Required:
- High school diploma or equivalent.
- Two (2) years' of recent experience in a healthcare environment; such as, working with medical information or work in a physician office or other clinical setting.
- Valid state driver's license, reliable transportation and automobile insurance required and maintained throughout employment.
Preferred:
- Completion of a Medical Assistant training program.
- Associate degree in business or health care field.
- Two (2) years' experience in working in population health.
